The Extenuating Circumstances Application form is available to download from the Trinity Saint David website via the following link EC application form under Chapter 13: Student Cases.
Once you have downloaded the form you will need to complete sections 1 – 7, sign and date the form, making sure you have read the guidance notes at the end of the document. Please see below for more detailed guidance on how to fill in your application.
Section 1
Please complete all the boxes in the personal and course details section, if you do not have some of the information please contact Student Support at Learna for further guidance.
Section 2
In this section, you must select the nature of the circumstance(s) that have affected/will affect your studies.
Section 3
Module Code & Name
To find this please use the UWTSD code and name, which can be found in your Programme of Study Handbook.
Assessment Type, Component & Weighting
Here you need to identify whether the work is coursework, portfolio, essay etc, if there is more than one piece of work for the module it is important that the component you wish to claim for is clearly shown together with the weighting it carries. Please use a separate line for each component.
Uncapped or Capped – Here you will need to identify if this is your first attempt at the work (uncapped) or if you are claiming a re-sit (capped).
Date work actually submitted – If the work was handed in within 7 days of the deadline, you will need to enter the actual date the work was submitted. Dates in the future should not be entered here
Evidence
You will be asked to submit support evidence for the extenuating circumstances claim, please click here and go to pages 5-7 for details on the required documentation.
Signature and date
Before your application is submitted, you must first sign and date the form. You may type your name in the box to sign the form electronically. Once this has been completed, your claim is ready to be emailed together with any supporting evidence to support@learna.ac.uk.
